      • How long does a hip replacement last?
        February 19, 2019A systematic review and meta-analysis of case series and national registry reports with more than 15 years of follow-up Total hip replacement is a common and highly effective operation. All hip replacements would eventually fail if in situ long enough and it is important that patients understand when this might happen. We aimed to answer the question: how long does a hip replacement last?Findings We identified 140 eligible articles reporting 150 series, and included 44 of these series (13 212 total hip placements). National joint replacement registries from Australia and Finland provided data for 92 series (215 676 total hip replacements). The 25-year pooled survival of hip replacements from case series was 77·6% (95% CI 76·0–79·2) and from joint replacement registries was 57·9% (95% CI 57·1–58·7). Interpretation Assuming that estimates from national registries are less likely to be biased, patients and surgeons can expect a hip replacement to last 25 years in around 58% of patients.       • Call-back of pacemakers
        February 14, 2019Many patients with certain pacemaker will have a call-back  anonymous Patients with Medtronic pacemaker Adapta, Versa, Sensia, Relia, Attesta and Sphera and Vitatron series A, E, G and Q have a call-back. The affected devices have been sold in between 10.03.2017 and 07.01.2019. Out of 156.957 devices there were records from 2 patients about 4 disfunctions. In a certain modus the pacemaker may pause stimulating. The problem may be solved by programming. Read the Medtronic call-back here.       • Stem cell-derived mouse kidneys
        February 5, 2019Generation of pluripotent stem cell-derived mouse kidneys in Sall1-targeted anephric rats Goto T et al. Regeneration of human kidneys in animal models would help combat the severe shortage of donors in transplantation therapy. Previously, we demonstrated by interspecific blastocyst complementation between mouse and rats, generation of pluripotent stem cell (PSC)-derived functional pancreas, in apancreatic Pdx1 mutant mice. We, however, were unable to obtain rat PSC-derived kidneys in anephric Sall1 mutant mice, likely due to the poor contribution of rat PSCs to the mouse metanephric mesenchyme, a nephron progenitor. Here, conversely, we show that mouse PSCs can efficiently differentiate into the metanephric mesenchyme in rat, allowing the generation of mouse PSC-derived kidney in anephric Sall1 mutant rat. Glomerular epithelium and renal tubules in the kidneys are entirely composed of mouse PSC-derived cells expressing key functional markers. Importantly, the ureter-bladder junction is normally formed.       • Efficacy and Safety of Vertebral Augmentation
        February 1, 2019The Efficacy and Safety of Vertebral Augmentation: A Second ASBMR Task Force Report PR Ebeling et al There is little to no evidence that two surgical procedures used to fuse crumbled vertebrae following a spinal fracture caused by osteoporosis reduce pain for patients any better than non-surgical or placebo procedures, according to a new report from a global task force of bone health experts published today in the Journal of Bone and Mineral Research (JBMR). Key findings from the task force report include: Vertebroplasty (cement injection into the vertebrae) provides no clearly significant benefit in pain control over placebo or sham procedures based on five randomized placebo-controlled trials. For balloon kyphoplasty, the lack of placebo-controlled trials of this procedure, along with the absence of any benefits of kyphoplasty over vertebroplasty when compared in a small number of head-to-head trials, argues against the use of this procedure. The ASBMR task force offered the following guidance for healthcare professionals and their patients for managing fractures to the spine: Fully inform their patients of the available evidence: there is little to no evidence that the use of vertebral augmentation works any better than a placebo. Anti-osteoporosis medications should be started or continued. A change in treatment may also need to be considered if the fracture occurred after 12 months from starting anti-osteoporosis treatment.
