      • Mesh versus suture repair of umbilical hernia in adults
        March 3, 2018Mesh versus suture repair of umbilical hernia in adults: a randomised, double-blind, controlled, multicentre trial Kaufmann R et al. Findings Between June 21, 2006, and April 16, 2014, we randomly assigned 300 patients, 150 to mesh repair and 150 to suture repair. The median follow-up was 25·1 months (IQR 15·5–33·4). After a maximum follow-up of 30 months, there were fewer recurrences in the mesh group than in the suture group (six [4%] in 146 patients vs 17 [12%] in 138 patients; 2-year actuarial estimates of recurrence 3·6% [95% CI 1·4–9·4] vs 11·4% (6·8–18·9); p=0·01, hazard ratio 0·31, 95% CI 0·12–0·80, corresponding to a number needed to treat of 12·8). The most common postoperative complications were seroma (one [<1%] in the suture group vs five [3%] in the mesh group), haematoma (two [1%] vs three [2%]), and wound infection (one [<1%] vs three [2%]). There were no anaesthetic complications or postoperative deaths. Interpretation This is the first study showing high level evidence for mesh repair in patients with small hernias of diameter 1–4 cm.         March 1, 2018Behavioural outcomes of subthalamic stimulation and medical therapy versus medical therapy alone for Parkinson’s disease with early motor complications (EARLYSTIM trial): secondary analysis of an open-label randomised trial Lhommée E et al. In a large cohort with Parkinson’s disease and early motor complications, better overall behavioural outcomes were noted with bilateral subthalamic stimulation plus medical therapy compared with medical therapy alone. The presence of hyperdopaminergic behaviours and neuropsychiatric fluctuations can be judged additional arguments in favour of subthalamic stimulation if surgery is considered for disabling motor complications.         February 23, 2018Satisfaction with Knee-TEP depends mostly from expectations, grade of functional improvement and pain course. Roland Fath Laut einem systematischen Literatur-Review mit Daten von 2005 bis 2016 hängt die Zufriedenheit von Patienten mit Knie-TEP in erster Linie von den Erwartungen vor dem Eingriff, vom Ausmaß der Funktionsverbesserung und dem Schmerzverlauf ab (2). Bei korrekter Indikation, chirurgisch einwandfreiem Eingriff sowie komplikationslosem Verlauf können derartige Implantate 10 bis 15 Jahre und länger halten. In Registerstudien wurden für Knie-TEPs Standzeiten von 97 %, 89 % und 78 % nach 5, 10 und 15 Jahren angegeben (3). Hauptgründe für frühzeitige Wechsel sind Infektionen, Patellakomplikationen, Fehlpositionierung der Endoprothese sowie Endoprothesenlockerung. Erhöht ist das Risiko für ein frühzeitiges Implantatversagen bei Patienten mit starkem Übergewicht und Begleiterkrankungen wie Gicht, Diabetes oder Rheuma. Auch gefürchtete Infektionen der Prothese drohen bei Vorliegen von Übergewicht oder Begleiterkrankungen häufiger. Während das Risiko einer Protheseninfektion bei jüngeren gesunden Patienten nach Angaben von Windhagen nur 1 : 250 (0,4 %) beträgt, liegt es bei einemDiabetiker rund 10-fach höher (4 %) und bei einem übergewichtigen Diabetiker bereits bei 8 % (1 : 12). Schwere Komplikationen sind nach Implantation einer Knie-TEP selten. Laut internationalen Daten liegt die intrahospitale Ereignisrate nach endoprothetischer Versorgung des Kniegelenks zwischen 1,5 % und 3,9 % (3). Gefäßverletzungen treten mit einer Häufigkeit von 0,08 % bzw. bis zu 0,36 % auf (bis 30 Tage nach dem Eingriff), sensorische bzw. sensomotorische Nervenstörungen in 0,2 % (akutstationärer Aufenthalt) bis 0,65 % der Fälle (< 3 Monate).
